We are seeking an experienced and compassionate Specialist Palliative Care Social Worker (Band 7) to join the Adult Community Services Specialist Palliative Care Team for the next 6 months.
This rewarding and specialist role focuses on providing expert social work support to people living with life-limiting illness, and their families and carers, during times of significant change, uncertainty, and loss.
Working as a core member of the multidisciplinary Specialist Palliative Care Team, you will collaborate closely with Clinical Nurse Specialists, Consultants and other professionals to deliver holistic, person-centred support across a range of care settings.
Key Responsibilities
Clinical Practice

Provide specialist social work support to adults with life-limiting illness and their families.

Undertake comprehensive psychosocial assessments and care planning.

Deliver complex emotional and practical support to service users and carers.
-
Support individuals to remain safely at home where possible.

Assist with transitions into residential or nursing care where required.

Provide advice and signposting on housing, benefits, finances and community services.

Advocate for service users and families with complex needs.

Contribute to holistic multidisciplinary assessments and care planning.

Respond to referrals to the Specialist Palliative Care Team.
Multidisciplinary Working

Work as an integral member of the Specialist Palliative Care Team.

Collaborate with consultants, nurses, GPs and other professionals.

Provide specialist social work advice to the wider Adult Community Health & Social Care Team.

Build strong working relationships with partner agencies and service providers.

Participate in multidisciplinary meetings and case discussions.
