We are seeking an experienced and motivated Senior Team Leader – Community Social Care to join our Community Health and Social Care Services team for a three month block booking.
Working closely with the Social Care Team Coordinator, you will play a key role in the day-to-day leadership and delivery of high-quality care services, supporting individuals with complex needs to live as safely and independently as possible within their own homes.
Key Responsibilities
Clinical and Service User Care
-
Assess service users with complex health and social care needs.
-
Develop, implement and evaluate person-centred care plans.
-
Work autonomously and recognise when escalation is required.
-
Promote independence, safety and wellbeing within the community.
-
Provide health promotion and education to service users and families.
-
Ensure safeguarding and risk management procedures are followed.
-
Maintain accurate clinical and care records.
-
Support safe and effective care delivery across the service.
Essential Requirements
Qualifications

Registered Nurse (NMC) or Registered Social Care Professional

Health and Social Care Degree
-
Evidence of Continuing Professional Development (Diploma level or equivalent)
