We are seeking an experienced and motivated Senior Team Leader – Community Social Care to join our Community Health and Social Care Services team for a three month block booking.

Working closely with the Social Care Team Coordinator, you will play a key role in the day-to-day leadership and delivery of high-quality care services, supporting individuals with complex needs to live as safely and independently as possible within their own homes.

 

Key Responsibilities

Clinical and Service User Care

  • Assess service users with complex health and social care needs.

  • Develop, implement and evaluate person-centred care plans.

  • Work autonomously and recognise when escalation is required.

  • Promote independence, safety and wellbeing within the community.

  • Provide health promotion and education to service users and families.

  • Ensure safeguarding and risk management procedures are followed.

  • Maintain accurate clinical and care records.

  • Support safe and effective care delivery across the service.

 

Essential Requirements

Qualifications

  • Registered Nurse (NMC) or Registered Social Care Professional

  • Health and Social Care Degree

  • Evidence of Continuing Professional Development (Diploma level or equivalent)

Skills and Experience

  • Experience working with individuals with complex needs

  • Strong leadership and team management skills

  • Excellent communication skills

  • Ability to work autonomously and as part of a multidisciplinary team

  • Strong organisational and prioritisation skills

  • Good IT and administrative skills

  • Ability to manage challenging situations calmly and professionally

  • Commitment to personal and team development
