We are seeking an experienced Community CNS to join a specialist palliative care service in the Channel Islands, supporting patients with life-limiting illness in a community-based setting.
As a CNS in palliative care, you will work both autonomously and as part of a highly skilled multi-disciplinary team to deliver high-quality, holistic care that meets the physical, emotional, social and spiritual needs of patients at all stages of their palliative journey.
About the Role

Provide expert clinical assessment, symptom management, and psychological support to patients with complex palliative care needs

Act as a resource for patients, families, carers and professionals, offering specialist advice and guidance

Facilitate advanced care planning and shared decision-making to ensure patient-centred goals of care

Respond to end-of-life care emergencies and complex symptom presentations in the community

Contribute to service development, audit and quality improvement initiatives

Support and educate colleagues through formal and informal teaching and supervision

Promote and embed best practice in palliative and end-of-life care across community settings
Essential Criteria:

Registered 1st level nurse with current NMC registration

Degree in a Health and/or Social Care related subject

Palliative Care award or qualification
-
Evidence of working towards a postgraduate qualification/award in Palliative Care (or equivalent)

Commitment to completing a Non-Medical Prescribing qualification

Advanced Communication Skills Training completed

Significant post-registration experience, with substantial experience in a palliative care setting

Expert knowledge of symptom management and end-of-life care emergencies

Experience of providing informal teaching and support to colleagues
-
Understanding of audit, service evaluation, and benchmarking in clinical practice

Strong knowledge of current professional and political issues in palliative and end-of-life care

Clear understanding of clinical governance and its role in safe, effective care
