Are you a Senior Pharmacist with experience in prescribing support unit?  look no further! Renowned Hospital in Channel Islands are looking for long term staff.

JOB SUMMARY:

 

Work with the Prescribing Advisor and independently to provide high quality evidence-based, clinical and technical support so that clinically appropriate and cost-effective prescribing is maintained and further improved.

 

Working in partnership with prescribing organisations and prescribers in primary and secondary care.

 

To provide clinical pharmacy services and medicines optimisation within the  Hospital Pharmacy Team (1 day).

 

RELATIONSHIPS:

 

Medical, Nursing and Pharmacy staff in primary and secondary care.

The Chief Pharmacist, Deputy Chief Pharmacist, Pharmacy Hospital Services Manager, Head and Lead Pharmacy Technicians and all other Pharmacy staff. Management staff, external agencies, patient groups.

 

MAIN DUTIES AND RESPONSIBILITIES:

 

The provision of a high-quality service, delivered in professional and courteous manner is a requirement of all staff employed by the Pharmaceutical Service.

 

  • To provide clinical support to the Prescribing Support Unit and to prescribing and dispensing organisations in primary and secondary care.

 

  • To supply prescribing data for revalidation and appraisal purposes to medical and non-medical prescribers.

 

  • To support the reintroduction of an Electronic Prescribing Decision Support System, in Primary Care as a means of ensuring that high quality prescribing advice is delivered at the point of prescribing in Primary Care.

 

To agree the recommendations with stakeholders, to write the profile, to monitor the acceptance rate of recommendations and to make regular checks to ensure that costs quoted are correct and up to date.

 

  • To support the use of other new technology to support prescribing of for example anti-infective agents, the Prescribing List on the States internet page.

 

  • To liaise with drug companies to ensure that HSC can access all Patient Access Schemes which are available to the NHS.

 

  • To implement, record, manage and update rebate schemes.

 

  • Using the EPACT system and NHSBSA outputs, to monitor, interpret and act on all aspects of prescribing, dispensing and charging via the Pharmaceutical Benefit system.

 

  • To support the communication of decisions or recommendations of the Prescribing Panel to applicants and to the wider community.

 

  • To support a programme of regular updates for pharmacy, medical and nursing staff, including face to face events, online meetings, guidelines and prescribing bulletins.

 

  • Promoting positive and effective communication within the Pharmacy Department.

 

  • To act as the Responsible Pharmacist in accordance with General Pharmaceutical Council (GPhC), legislation and departmental SOPs

 

  • Establish links with other pharmacists in order to share / learn from examples of best practice.

 

  • To adhere to and ensure that all staff follow HSC and departmental Health and Safety, COSHH guidelines and departmental procedures

 

  • Undertake any other training and duties as required.

 

  • To participate in the Saturday, pharmacy bank holiday, flexible working weekend and evening Rota’s, as required to meet the needs of the service

 

  • Receive, store and supply Controlled Drugs in accordance with the Misuse of Drugs Act.

 

  • Contribute to the effectiveness of the pharmaceutical service by putting forward suggestions for innovation and development in professional, technical and managerial aspects of the work.

 

  • To participate and contribute to Patient Safety Meetings and where appropriate attend Clinical Risk Meetings.

 

  • To practice at all times in accordance with the General Pharmaceutical Council (GPhC) code of ethics.

 

  • To undertake any other duties of a similar nature when requested to by the Pharmacy Manager or their deputy.

 

  • To comply with the principles contained in the Data Protection Law 2001, concerning improper disclosure, misuse or breach of confidentiality in respect of information held on computer systems or otherwise.

 

CLINICAL GOVERNANCE

 

  • To report and to investigate clinical incidents involving all aspects of prescribing, dispensing and administration of medication, particularly in Primary Care and at the interface, ensuring processes are in place to develop learning from these incidents

 

  • Assist in the investigation of complaints and errors, in conjunction with the Prescribing Advisor, to resolve any issues as close to source as possible

 

  • To prepare, to interpret and to share prescribing data at island, prescribing organisation, prescriber and dispenser level.

 

  • To facilitate the undertaking of and contribute to multi-disciplinary audit.

 

  • To contribute, monitor and act on departmental and HSC Key Performance Indicators and Benchmarking.

 

LEADERSHIP

 

  • To collaborate on transformation of and innovation in service delivery.

 

  • To attend and advise relevant HSC committees, recommend mechanisms for internal and external audits as required providing advice to medical staff on medicines usage and expenditure.

 

  • To be a member of the HSC Prescribing and Formulary Committee.

 

SERVICE DEVELOPMENT

 

  • To support the development of business cases for new service developments in the pharmaceutical service and for new medicines.

 

  • To ensure the effective use of the pharmaceutical budget including salaries, equipment, travelling and training budgets.

 

  • All staff will support their managers to make efficient and effective use of resources.

 

  • All staff are responsible for identifying any actual or potential deviation from budgets and are to work with the budget holder or manager to find effective ways of handling it.

 

  • All staff must ensure they use resources in a manner consistent with organisational objectives and policies, ensuring that resources are realistic, justified and of clear benefit to the organisation

 

ADMINISTRATION

 

  • To ensure that all pharmacy paperwork and documentation is completed clearly and accurately and filed and archived appropriately.

 

  • To ensure that all Patient Records are maintained within the bounds of Professional standards, and Data Protection Laws.

 

  • To ensure that all monitoring and audit data is recorded and reported upon in a timely fashion and audit trails are maintained and accessible.

 

  • To ensure that all clinical incidents are reported and acted upon in a time frame as laid out in HSC Guidelines.

 

  • To ensure that all drug chart endorsements and transcriptions are done in a neat and readable manner.

MINIMUM QUALIFICATIONS

 

  • Masters Degree in pharmacy or equivalent.
  • General Pharmaceutical Council Registration.
  • Hold or be working towards a post-graduate diploma/masters degree or equivalent relevant to pharmacy practice.

  1. Demonstrable post registration in hospital or community pharmacy.

  3. Evidence of Medicines Optimisation experience.
  4. Experienced in Multidisciplinary Team working.

KEY COMPETENCIES:

  • To be part of the Prescribing Support Unit and to work alongside the Prescribing Advisor on query-answering, provision of prescribing data, reports, meetings, bulletins, guidelines, updates and audits.

 

  • To deputise for the Prescribing Advisor in their absence.

 

  • To engage with medical prescribing organisations to provide high quality evidence-based, clinical and technical support so that clinically appropriate and cost-effective prescribing is maintained and developed.

 

  • To investigate opportunities for safe, evidence-based and clinically appropriate cost reduction in prescribing.

 

  • To support the negotiation and on-going management of rebate schemes on products dispensed in Primary Care.

 

  • To support the management of the entry of new drugs in the community and in hospital.

 

  • To support the ongoing implementation of NICE Technology Appraisals in primary care as TA implementation moves to being part of business as usual.

 

  • To support and implement local medication policy changes, including NICE national guidance, recommendations of the Prescribing and Formulary Pane and NICE technology appraisals.

 

  • To support the re-introduction of an Electronic Prescribing Decision Support System, in Primary Care.

 

  • To support the introduction of electronic prescribing in HSC and any interface/discharge services.

 

  • To support the Cannabis licensing team and to, in time, become an authorised signatory.

 

  • To be a member of the Patient Group Directive (PGD) group.

 

  • To support the possible expansion of the high cost drugs service.

 

  • To work in the Prescribing Support Unit with approximately 1 day per week clinically in the PEH Pharmacy.
