Social Workers – London Boroughs

3x Band 7 Social worker specialists needed to assist 3 London Borough

Provide urgent social work assessments to support admission avoidance and safe discharge
Respond rapidly to individuals in crisis within A&E and community settings.
Assess care, support and safeguarding needs in line with Care Act requirements.
Coordinate timely social care interventions to facilitate safe care at home.
Work collaboratively with health, social care and voluntary sector partners.
Identify risks and implement safeguarding measures to protect vulnerable adults.
Support patients and families to make informed decisions regarding care options.
Facilitate hospital discharge planning and prevent avoidable admissions and readmissions.
Arrange care packages, community support services and short-term crisis interventions.
Maintain accurate records and ensure compliance with statutory and professional standards.

NHS experience required – post qualification as a PT highly desireable

Car drivers required given the community role across 3 London Boroughs
